General Terms and Conditions of Co-operation
Dear Sirs,
We are very pleased with the interest you have shown in our Company as a potential business partner. However, before we can discuss the details of co-operation, please read the following documents, the acceptance of which is necessary to start it:
- General Conditions of Carriage, available here
- Sample Order for Carriage of Goods which we use when ordering transport (available as an attachment on this page).
- GDPR information for Contractors and their employees/associates and information on the Joint Administration of personal data: Protection of personal data of Suppliers entering into cooperation with Alupol Packaging S.A.
In addition, please download and carefully complete the questionnaire available as an attachment on this page and send it to: mjakut@grupakety.com.
The questionnaire should be filled out digitally and sent as a .pdf file – do not print. Please include also:
- Licenses: domestic, export
- Third Party Liability for Road Carrier Insurance – in full (prerequisite; sensitive data can be blacked out)
- Current copy of KRS/CEiDG/REGON
By submitting the completed questionnaire and the listed documents, you confirm that you and the persons whose data are shared with us have become familiar with the above-mentioned GDPR information.
Please include the word “co-operation” in the subject of the message.
After verification of the complete set of documents, you will receive information on the next steps.
If the above requirements are not met, no additional contact will be made.
